We prefer the supine position as it allows for ease of positioning the patient on the operating room table and anatomic orientation of the patient s pelvis which minimizes the potential for implant malposition and allows for easier leg length restoration. The approach is essentially the same as the kocher langenbeck but exposure is limited to the hip joint respecting but not displaying the sciatic nerve. In this position the pelvis may have various degrees of tilt leading to implant malposition. The patient is placed in a lateral decubitus position and the limb draped so that it can be moved during surgery. Most traditional approaches to total hip arthroplasty employ the lateral decubitus position but anterior total hip arthroplasty offers many advantages in the hands of an experienced surgeon.
